You may submit your claim online or use a paper form.

Please note that all materials submitted to the Claims Commission may become public records under the Arkansas Freedom of Information Act. If your claim contains sensitive personal information, including medical information, social security numbers, tax identification numbers, financial account information, it is your responsibility to remove or redact that information prior to submission. The Claims Commission is not responsible for any damages that may arise should you fail to remove or redact such information.

Failure to comply with all filing requirements may result in the return of your claim documents to you, which may delay the processing of your claim.
